Job summary

Grace Pointe in Greeley, CO (1919 68th Avenue) seeks PRN QMAPs to join the Assisted Living/Memory Care team. You will help meet resident needs and administer medications as ordered by the attending physician, under supervision of the Charge Nurse and facility policies.

Qualified candidates must hold a QMAP certification. The salary range is $17.00-$19.00 per hour, based on experience, with shift differentials for nights and weekends.

Job description

Grace Pointe, a premier Senior Care Community located at 1919 68th Avenue in Greeley, CO, has openings in our Assisted Living/Memory Care Department for PRN QMAPs. The primary purpose of your job position is to provide services to meet the needs of the residents in Assisted Living/Memory Care, and to assist in the administration of medications to residents as ordered by the attending physician, under the direction of the attending physician, and the supervisor. You must have a QMAP certification for this position. The salary range is $17.00-$19.00 per hour, based on years of experience.

Shift Differentials:

• Monday - Friday, 6pm-6am (NIGHTS): additional $2.50/hr

• Saturday-Sunday, 6pm-6am (NIGHTS): additional $3.50/hr

• Saturday-Sunday, 6am-6pm (DAYS): additional $2.50/hr

Job duties include but are not limited to:

• Answer resident call lights and assist them with their needs.

• Follow service plan for each resident to ensure needs are met. Assist residents with activities of daily living per the service plan.

• Remind residents of daily activities as appropriate and escort them to the activity if needed.

• Welcome residents to the dining room and serve drinks and meals. Follow up with each resident to ask if they need anything else. Help to seat residents at their tables.

• Deliver meals to individual apartments as appropriate.

• Clean residents’ rooms and common areas as needed to include removing trash, returning dishes to the kitchen, vacuuming/floor clean-up, cleaning bathrooms, tidying up kitchenettes, making the bed, etc.

• Provide laundry / linen service for the residents per the service plan.

• Report any significant change in resident's baseline status, any signs of infection, injury, or exposure to communicable disease to the Charge Nurse of Assisted Living/Memory Care by documenting in the communications section in Point Click Care.

• Complete the task list for your shift and submit the completed task list to the Charge Nurse of Assisted Living/Memory Care at the end of the shift.

• Accurately and safely prepare, administer, and document the oral and/or topical medications that are commonly used in the facility and that may be ordered for resident use by the attending physician.

• Verify that any medications brought into the facility by a newly admitted resident are examined and identified by the Director of Nursing or Designee.
