Job summary

Envision Unlimited is seeking a Qualified Intellectual Disabilities Professional (QIDP) in Illinois to coordinate comprehensive case management services for members in community living settings. The role involves developing individualized programs, documenting progress, and collaborating with guardians, staff, and health providers to meet compliance and service goals.

The QIDP will maintain member records, ensure benefit entitlements, participate in planning meetings and discharge/transfer

Qualifications

• Bachelor's degree in Human Service field is required.
• Must be a QIDP; verification letter required.
• Excellent written and communication skills, computer, and organizational skills.
• Direct Service Professional training and STOFHLA test required.
• Valid Illinois driver's license and ability to travel as needed.
• Criminal background check and Nurse Aide Registry clearance.

Responsibilities

Summary:

The Qualified Intellectual Disabilities Professional (QIDP) is responsible for ensuring members receive comprehensive case management services in community living settings. The QIDP coordinates, implements, and documents individualized programs and services to support members with intellectual and/or developmental disabilities in achieving their highest level of independence. This position serves as a liaison among members, guardians, staff, healthcare providers, and other stakeholders to ensure compliance with State, Federal, and agency requirements.

Essential Responsibilities

• Develop and implement individualized, progressive programs for members with intellectual and/or developmental disabilities in community living settings.

• Schedule, assign, document, and monitor member activities to ensure goals and outcomes are implemented.

• Develop Implementation Plans and complete required assessments, including ICAP and SLOF.

• Ensure accurate and timely completion of CIRAS, incident, and hospitalization reports in the electronic health record system.

• Submit required documentation to Human Rights Committee and Behavior Management Committee and attend meetings as required.

• Ensure members receive all benefits to which they are entitled and coordinate documentation related to individual finances.

• Coordinate professional and consultative services and ensure implementation of recommended services.

• Attend member psychiatric and medical appointments as needed.

• Ensure compliance with mandated reporter and OIG requirements and cooperate fully with investigations.

• Maintain current, accurate member records and attendance reports in compliance with State, Federal, and agency guidelines.

• Compile, write, and monitor member outcomes.

• Participate in Discovery Process, Person-Centered Planning meetings, and special staffings as required.

• Complete documentation in the agency’s electronic record system and support staff training on proper system use.

• Maintain complete and accurate case records, including monthly progress documentation.

• Communicate regularly with parents, guardians, visitors, and community agencies and document contacts as required.

• Collaborate with nursing and direct support staff regarding medical appointments, medications, and follow-up care.

• Respond to emails and phone calls in a timely manner and maintain appropriate out-of-office notifications when applicable.

• Facilitate and document discharge or transfer planning and coordinate member moves as needed.

• Maintain ongoing communication with healthcare providers and guardians during hospitalizations and conduct hospital visits as required.

• Assume periodic on-call responsibilities and respond to emergencies as needed.

• Maintain confidentiality in accordance with the Illinois Mental Health Code and Confidentiality Act.

• Maintain a professional image as a representative of Envision Unlimited.

• This position may be required to work evenings, weekends, or flexible hours based on program needs.

SUPERVISORY RESPONSIBILITIES

No supervisory responsibilities.

EDUCATION, SKILLS, and/or EXPERIENCE

• Bachelor's degree in Human Service field and a minimum of three years experience in the field of developmental disabilities.

• Must be a QIDP. Verification letter is required.

• Must have excellent written and communication skills, computer, and organizational skills.

• Must have proof of Direct Service Professional training and pass STOFHLA test.

• Must have valid Illinois driver's license and the ability to travel per needs of the program.

• Must pass Criminal background check and Nurse Aide Registry.
