Job summary

Envision Unlimited seeks a compassionate, organized Qualified Intellectual Disabilities Professional Case Manager to support adults with developmental disabilities in living full, meaningful lives.

You will build relationships, coordinate services, and advocate for clients while maintaining records and collaborating with families and teams. Travel across Cook County is ~70% to support community-based services.

Qualifications

• QIDP-eligible at hire; minimum 1 year working with adults with developmental disabilities.
• Bachelor's degree in Human services or related field.

Responsibilities

• Coordinate service agreements, authorizations, and referrals.
• Develop and implement strategies with families and support teams.
• Maintain electronic records and case notes (Quantum).
• Provide advocacy and support during crises and transitions.
• Partner with community resources and internal teams for holistic care.
• Travel throughout Cook County for client visits (about 70% in the community).

What We’re Looking For:

Must be QIDP-eligible upon hire which includes:

• At least 1 year working with adults with developmental disabilities.
• Bachelor's degree in Human services field, including, but not limited to Sociology, Special Education, Rehabilitation Counseling or Psychology, Recreation or in a specialty area such as Art, Dance, Music or Physical Therapies.

Also required:

• Valid driver’s license and reliable vehicle.
  
• Flexibility and willingness to travel locally (mileage and gas reimbursed).
