Job Description: Quality Assurance & Performance Improvement (QAPI) Specialist

This role involves assisting with agency efforts to improve clinical documentation and outcomes through a systematic, comprehensive, and data-driven approach to maintaining and enhancing safety and quality. Responsibilities include compiling trends, reporting quality data across various domains, developing performance improvement plans, conducting record reviews, data analysis, and serving as a regulatory resource for clinical staff. Success requires a passion for quality clinical care and the ability to share knowledge effectively.

Key duties include:

  • Promoting understanding, communication, and collaboration within teams to ensure best practices are utilized.
  • Participating in the development, implementation, and evaluation of clinical quality improvement action plans.
  • Engaging in agency QAPI activities, including monitoring patient and employee incidents, infection tracking, and quality indicators such as OASIS and patient satisfaction.
  • Assessing clinical staff compliance with internal policies and external standards.
  • Assisting with quality improvement data analysis and reporting.
  • Reviewing OASIS assessments for accuracy and compliance with regulations and policies.
  • Ensuring proper ICD-10 coding related to patient conditions.
  • Developing interventions to improve quality scores and participating in initiatives to enhance quality metrics.
  • Reviewing corporate data and promoting culturally competent performance improvement plans.
  • Performing clinical record audits and providing educational feedback.
  • Facilitating the use of evidence-based guidelines to reduce practice variation and optimize outcomes.
  • Ensuring compliance with all regulatory and organizational requirements, including timely data submissions.
Qualifications
  • Associate of Science Degree required.
  • Bachelor's degree in nursing, PT, OT, SLP, MSW, Health Administration, Management, or related discipline preferred.
  • Minimum of 3 years of certified home health experience required.
  • Experience with Value-Based Purchasing, PDGM, CAHPS preferred.
  • Licensure in Connecticut in your field of specialty.
  • Experience in data analysis, statistical processes, and quality reporting.
  • Ability to design and develop performance measures and provide education.
  • Experience facilitating PI teams using quality improvement methodologies.
  • Excellent communication skills, both verbal and written.
  • Willingness to travel to regional branches; travel within the U.S. may be necessary.
  • Knowledge of federal and state regulatory requirements.
