Qualifications

• Active RN or LVN license required.
• Minimum 2 years of home health experience preferred.
• Experience with Kinnser/WellSky EMR preferred.
• Strong knowledge of Medicare documentation guidelines.
• OASIS experience required.
• Excellent written and verbal communication skills.
• Strong critical thinking and analytical skills.
• High attention to detail and ability to meet deadlines.

Responsibilities

• Review clinical documentation in Kinnser (WellSky) for accuracy, completeness, and compliance.
• Ensure documentation meets Medicare, state, and accreditation standards.
• Audit OASIS assessments and skilled nursing notes.
• Identify documentation deficiencies and provide constructive feedback to field clinicians.
• Collaborate with Clinical Managers to ensure timely corrections.
• Monitor trends in documentation and recommend process improvements.
• Assist in preparing for surveys and audits.
• Maintain confidentiality and HIPAA compliance at all times.
