Job summary

Veterans Health Administration, Department of Veterans Affairs in Reno, NV seeks a nurse practitioner to provide independent patient care in pulmonary/sleep clinics, coordinating with a multidisciplinary team.

The role includes evaluating patients, performing exams, coordinating care plans, teaching patients, and participating in quality improvement and clinical research; schedule Monday to Friday, 8 am to 5 pm; telework Ad-Hoc with supervisor approval.

Qualifications

• Independent practice without physician supervision.
• Ability to coordinate multidisciplinary pulmonary/sleep care.
• Strong collaboration with physicians, nurses, psychologists and other staff.

Responsibilities

• Coordinate pulmonary/sleep patients and multidisciplinary staff in outpatient and inpatient care, including patient evaluation and triage.
• Provide teaching to patients and family as needed.
• Serve as consultant to multidisciplinary staff in problem-solving for complex care plans.
• Develop and utilize clinical guidelines to streamline pulmonary/sleep care.
• Act as a stable point of contact for pulmonary/sleep patients throughout the outpatient process.
• Perform history and physical exams in outpatient clinics and assist in treatment planning.
• Make assessments and referrals to meet patient needs.
• Facilitate consultations and follow-up care for pulmonary/sleep patients.
• Collaborate with physicians, nurses, psychologists, social workers, pharmacists, dietitians, chaplains, and cancer program staff to meet multidisciplinary needs.
• Facilitate patient participation in clinical research with study coordinators and research nurses.
• Participate in quality improvement activities and monitor treatment guidelines.
• Other duties as assigned.
