Job summary

Alteas Health is seeking a Nurse Practitioner to deliver specialty Pulmonology care to patients in Skilled Nursing, Long-Term Care, and Post-Acute settings across St. Paul, St. Louis Park, Roseville area.

You will collaborate with physicians and care teams to provide high-quality, patient-centered services. The role offers clinical autonomy with physician collaboration and a Monday–Friday daytime schedule, no regular weekends or on-call responsibilities, supporting work-life balance.

Qualifications

• Current, unrestricted NP license in applicable state(s).
• National certification appropriate to licensure.
• Current BLS certification.
• Minimum six months of NP clinical experience.
• Valid driver’s license and reliable transportation.
• Ability to travel daily throughout an assigned territory.
• Experience with Electronic Medical Records (EMR).

Responsibilities

• Evaluate, diagnose, and manage pulmonary conditions across care settings.
• Develop individualized treatment plans using evidence-based practice.
• Order and communicate lab tests, imaging, and diagnostics.
• Prescribe medications within scope of practice.
• Educate patients and families about diagnoses and care plans.
• Coordinate care with physicians and interdisciplinary teams.
• Maintain accurate clinical documentation and rounding at facilities.
• Build strong relationships with facility leadership and teams.
