Alteas Health seeks Nurse Practitioners or Physician Assistants to deliver specialty Cardiology or Pulmonology care in Skilled Nursing, Long-Term Care, and Post-Acute Care settings in the Champaign area.

Qualifications

• Current, unrestricted NP/PA license in applicable state(s).
• Current national certification appropriate to licensure.
• Current Basic Life Support (BLS) certification.
• Minimum six (6) months NP/PA clinical experience.
• Valid driver's license with reliable transportation.
• Ability to travel daily within assigned territory.
• Strong clinical judgment, communication, organizational, and critical thinking skills.
• Experience using Electronic Medical Records (EMR).

Responsibilities

• Evaluate, diagnose, and manage cardiovascular or pulmonary conditions.
• Develop individualized treatment plans using evidence-based practice.
• Order, interpret, and communicate lab tests and imaging results.
• Prescribe medications and treatments within scope.
• Educate patients and families on diagnoses and ongoing care.
• Coordinate care with physicians, nursing staff, and therapy teams.
• Maintain accurate and timely clinical documentation.
• Round at facilities in the assigned territory.
