Job summary

Yale NewHaven Health in Connecticut is recruiting a physician to provide non-surgical primary and continuing care within the Northeast Medical Group. The role includes leadership of an interdisciplinary team of primary caregivers and administrative staff, ensuring ethical practice and alignment with our mission, vision and values.

The physician will examine patients, order tests, develop treatment plans, coordinate care with referrals, and maintain accurate documentation while promoting access

Qualifications

• MD or DO with ABIM-recognized internal medicine residency or ABFM-recognized family medicine residency.
• Valid board certification in internal medicine or family medicine and medical licensure in Connecticut.
• BLS certification and DEA registration required.

Responsibilities

• Examines patients, obtains medical histories and treats acute and chronic illnesses.
• Orders, performs and interprets diagnostic tests.
• Involves patients and their families in care and treatment decisions.
• Creates treatment plans and completes orders in patient's charts.
• Collaborates on improvements in care delivery and service with staff, other physicians and management.
• Initiates appropriate specialty referrals.
• Provides clinical information to the interdisciplinary team in a clear and timely manner.
• Ensures that proper patient documentation is timely, accurate and complete.
• Works with practice management to ensure proper billing, coding and CPT code assignment.
• Achieves and maintains optimal patient access.
• Provides interdepartmental medical consultation as requested.
• Participates in the development of medical protocols, standard procedures and management of patient-care-related activities.
• Provides on-call coverage.
• Gives feedback to and accepts feedback from all staff in respectful manner.
• Acts in a positive and professional manner, complying with practice policies, procedures and federal and state laws.
• Performs other duties and responsibilities as assigned, including promoting health education with other health professionals.
