Job summary
Infinite Medical P.C. seeks a Pulmonary Nurse Practitioner/Physician Assistant to support patients in the PHM program across nursing homes in Texas. You will assess, diagnose, and manage respiratory conditions, coordinating care with a multidisciplinary team.
You will travel regularly to facilities, perform diagnostic tests, and implement personalized treatment plans with a focus on patient education and outcomes. A valid NP/PA license and pulmonology experience are required.
Qualifications
- 6+ months of experience in long-term care or skilled nursing facilities.
- Active and unrestricted NP/PA/Physician license.
- DEA license.
- CPR/BCLS certified.
- Master’s degree in Nursing.
- Graduated from an accredited NP program.
- At least 1 year of pulmonology or respiratory care experience.
- Local travel 100% required.
Responsibilities
- Conduct physical assessments and medical screenings for pulmonary conditions.
- Diagnose and treat respiratory illnesses such as asthma, COPD, and pneumonia.
- Develop individualized treatment plans considering history and lifestyle.
- Prescribe medications and therapies (inhalers, antibiotics, rehab programs).
- Educate patients and families on disease management and medication adherence.
- Perform spirometry, chest X-rays, and arterial blood gas analysis.
- Monitor progress and adjust treatment plans as needed.
- Travel to multiple nursing homes within the designated region for on-site care.
- Maintain a flexible schedule to optimize patient coverage.
