Job summary

Infinite Medical P.C. is seeking a Pulmonology Nurse Practitioner/Physician Assistant to join the Patient Health Management program. You will assess and manage patients with respiratory conditions, travel to multiple nursing homes in NY, and support on-site care under a flexible part-time schedule.

The role requires 6 months in long-term care, a valid NP/PA license (DEA and CPR/BLS), and an accredited NP program. prior pulmonology experience is preferred.

Qualifications

• Active and unrestricted NP/PA/Physician license.
• DEA license.
• CPR BCLS/BLS certified.
• Master's degree in nursing completed.
• Graduated from an accredited NP program.
• Minimum 6 months of experience in long-term care.
• Local travel 100% to nursing homes required.
• Experience with EMR systems (Sigma, Visual, PCC) preferred.

Responsibilities

• Conduct physical assessments of patients with pulmonary conditions.
• Diagnose and treat respiratory illnesses such as asthma, COPD, and pneumonia.
• Develop and implement individualized treatment plans considering medical history, current symptoms, and lifestyle.
