Job summary

Qualifications

• Bachelor's degree or equivalent in a human service-related field is required.
• 1–2 years of experience in the social service field preferred.
• Experience with human service programs and Centre County agencies is valued.
• Valid Pennsylvania driver's license and willingness to travel required.

Responsibilities

• Provides case management to individuals/families involved with County departments and local agencies.
• Offers support and referrals to those navigating the human service system.
• Assesses challenges and barriers to receiving services.
• Builds relationships with service providers to assist with food, financial help, homelessness prevention, and utilities.
• Advocates for individuals involved with the court system, mental health, or developmental issues.
• Performs client needs assessment, service coordination, advocacy, and case notes; ensures confidentiality.
