Job summary

Staffing Solutions Organization (SSO) seeks a compassionate Public Health Social Worker to support Maine families through CYSHCN and lead‑related programs. You will provide case management, connect families to services, and collaborate with hospitals, schools, and state agencies.

The role is hybrid with two in-office days at 286 Water Street, Augusta, and a typical schedule Monday–Friday, 8:00 AM–5:00 PM. Travel statewide may be required.

Qualifications

• Current licensure in Social Work, Nursing, Counseling, or related field.
• Experience in case management, care coordination, or family support services.
• Experience assessing family needs and connecting individuals to available resources.
• Knowledge of community services and support programs.
• Ability to work independently while collaborating across multidisciplinary teams.

Responsibilities

• Conduct family needs assessments and develop service connection plans.
• Coordinate referrals to medical, social service, housing, transportation, and support programs.
• Document family interactions, referrals, and case outcomes.
• Maintain collaborative relationships with healthcare providers and community partners.
• Support equitable access to healthcare and community services.
• Participate in home visits as needed.
• Assist families in overcoming barriers affecting child health and well-being.
