Qualifications

Responsibilities

• Lead a multidisciplinary team of RNs, community health workers and outreach staff.
• Oversee client case management ensuring care is coordinated with public health goals.
• Monitor staff performance and provide clinical and operational guidance.
• Prepare reports to meet grant requirements and program objectives.
• Develop partnerships and support outreach to improve access to public health services.
