Sabathani Community Center is seeking a contracted Public Health Registered Nurse to conduct comprehensive initial assessments for families newly enrolled in Sabathani's Non-Medical Family Home Visiting program. The contracted PHN plays a critical clinical role in the program, administering validated screening tools, identifying family strengths and needs, and ensuring families with urgent safety or mental health concerns receive an immediate, appropriate response. This is a contracted role and is well suited to a nurse who is comfortable delivering culturally responsive, trauma-informed care in participants' homes.
