Job description

This position serves as lead nurse coordinator and supervisor for the Care Management for At Risk Children (CMARC) ages 0 to 5 years old program. The CMARC program provides direct care management services for eligible children residing in Alamance County. This position supervises two social worker CMARC positions, which includes annual performance appraisals and work assignments with oversight from care management nursing supervisor.


The CMARC program provides services to Medicaid enrolled children 0-5 years old and functions as a part of a multidisciplinary Care Management Team. The primary purpose of the position is to promote an optimal level of functioning for children at risk for disabilities due to prematurity, congenital abnormalities, toxic stress and/or maladaptive family units through the provision of care management services. Program services include assessment, planning, facilitation, care coordination, evaluation, client/family counseling, referral to needed resources, as well as patient advocacy to address health care and preventative service needs of Medicaid enrolled children ages 0-5. The Care Management for At Risk Children program focuses on coordination of the patients’ medical, nutritional, educational, social and emotional needs with private and community resources to promote a healthy childhood and family unit

As part of the team, this position must be familiar with and aim to achieve the programmatic goals set forth by NC Medicaid and the designated prepaid health plans. Services are provided within the community; local physician practices and patient home visits are required for meeting program measures. This position will also utilize multiple electronic medical records and State electronic systems for the provision and documentation of care. This position collects, reviews, assembles and reports program data, provides input on staff and program performance, and maintains and updates program policy and procedures. This position will assist with orientation and training of new staff and student learners. This position may also be expected to provide care management services to hospitalized Medicaid children ages 0-5 transitioning into the home environment.

This position will also be required to staff emergency shelters as mandated by the State Public Health Office; assist with Public Health outbreak investigations and response, and perform additional duties as required by the Nursing Supervisor, Nursing Director, Medical Director and Health Director.

ESSENTIAL KNOWLEDGE, SKILLS, AND ABILITIES:

Thorough knowledge of and skill in the application of nursing theory, practice, principles, and techniques employed in the field of public health and related programs; for employees in expanded role positions, considerable knowledge of, and skill in obtaining a health history and performing physical examinations sufficient enough to determine if there is any deviation from normal; considerable knowledge of available resources and organizations and the ability to coordinate these as needed; general knowledge of current social and economic problems relating to public health, including health disparities; ability to plan, coordinate, and oversee the work of others; ability to deal tactfully with others and to exercise good judgment in appraising situations and make decisions; ability to work in partnership with patients and with other service providers to elicit needed information and to maintain effective working relationships; ability to record accurately services rendered and to interpret and explain records, reports, and medical instructions; considerable knowledge of and ability to apply the principles and practices of public health; adequate computer skills to allow communication, patient record documentation, and accessing of information. May be exposed to blood or body fluids in performance of job duties.

MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS:

Graduation from a four-year college or university with a B.S. in Nursing which includes a Public Health Nursing rotation and two years of Public Health nursing experience; or a Master’s in Public Health and graduation from a school of professional nursing and two years of professional nursing experience; or graduation from a school of professional nursing and three years of professional nursing experience including two years of Public Health nursing experience; or an equivalent combination of training and experience.

NECESSARY SPECIAL QUALIFICATIONS: 1: A current license to practice as a registered nurse in North Carolina by the NC Board of Nursing. 2: Valid N.C. driver's license and have access to a personal vehicle. License to practice as a registered nurse in NC (if nurse applicant).


PREFERRED QUALIFICATIONS:Knowledge and experience with Maternal-Infant Health/Pediatrics preferred. Fluent in both Spanish and English both oral and written preferred.
