Purpose

The Public Health Nurse 3 serving the Scales NeighborhoodNursing (SNN) program assesses health needs, delivers comprehensive nursingservices, and implements strategies to improve health outcomes for residentswithin a designated City of Milwaukee neighborhood.

Essential Functions

Care Coordination and Documentation

  • Provide direct nursing care, including health screenings,immunizations, and chronic disease management support.
  • Assist clients in navigating healthcare systems, linkingthem to appropriate services and resources.
  • Develop and implement individualized care plans, incollaboration with clients and multidisciplinary teams.
  • Maintain complete, clear, and accurate documentation,including case notes and treatment care plans.
  • Maintain knowledge of national, regional, state, and localresearch and public health practices.

Community Engagement and Collaboration

  • Conduct community health assessments to identify healthdisparities, resources, and needs within assigned neighborhoods.
  • Build and maintain relationships with community members, healthcareprovider organizations, businesses, other governmental agencies, andcommunity-based organizations (CBOs).
  • Attend community events and meetings to engage withresidents and represent the SNN Program.
  • Collaborate with Public Health Nurses, Public HealthEducators, and other health professionals to achieve shared goals.
  • Participate in community coalitions and advisory groupsrepresenting public health nursing.

Scales Neighborhood Nursing (SNN) Program

  • Contribute to the design and implementation ofneighborhood-specific health interventions aligned with programmatic goals.
  • Advocate for policies and programs that address healthliteracy, social determinants of health, and reduce health inequities.
  • Facilitate access to health information.
  • Collaborate with management in the planning and evaluationof the SNN program to measure programmatic effectiveness and to inform decisionmaking.
  • Report progress and outcomes to supervisors andstakeholders.
  • Provide guidance concerning incidents, errors, and otherpatient service issues.

Conditions of Employment
  • Work standard hours: 8:00 a.m.-4:45 p.m. Monday throughFriday.
  • May be required to participate in a 24-hour emergencyon-call rotation schedule, including weekends.
  • Must wear industry related personal protective equipment(PPE) such as gloves, facemask or face shield.
  • May travel outside the City of Milwaukee, occasionallyovernight, to attend training and conferences.
  • Must be able to lift and move up to 25 lbs.
  • May be exposed to infectious agents or toxins.
  • May be required to respond to an emergency or broad impactevent.

  • Bachelor of Science Degree in Nursing (BSN) from anaccredited nursing program. (Note: students in their final semester mayparticipate in the selection process but may not be hired until the degree hasbeen completed.)
  • Current permit or license to practice professional nursingin Wisconsin OR eligibility to receive such permitorlicense. Licensemust be obtained by the time of appointment and maintained throughoutemployment.
  • Two years of professional nursing experience related tocommunity health, preventative health services, or patient advocacy.
  • Valid driver's license and possession of a properly insuredvehicle for use on the job is required at time of appointment and throughoutemployment. Private auto allowance may be paid pursuant to Section 350-183 ofthe Milwaukee Code.

Desirable Qualifications
  • Master’s degree in a related field from an accreditedcollege or university.
  • Ability to provide interpretation in Spanish, Hmong, oranother language.
  • Experience leading, supervising, or managing health-relatedstaff, programs, and/or projects.
Knowledge, Skills, Abilities & Other Characteristics

Technical

  • Knowledge of nursing practice and patient health assessmentrelated to community and public health care.
  • Knowledge of the information and techniques needed todiagnose and treat injuries and diseases.
  • Ability to efficiently use database, spreadsheet, wordprocessing, presentation and case management information systems.
  • Knowledge of mathematics to accurately calculate dosages andtake various measurements.
  • Ability to read and interpret documents such as safety rulesand technical materials.
  • Ability to adhere to organizational standards, policies, andanti-racism principles in all professional activities.

Communication and Interpersonal

  • Ability to work cooperatively, effectively, and fairly withcoworkers and residents whose background may differ from one’s own.
  • Ability to build and maintain effective workingrelationships with a multi-cultural and multi-disciplinary staff, the public,various City of Milwaukee officials, and other agencies.
  • Ability to provide services in a culturally sensitivemanner.
  • Ability to speak effectively to individuals and to groupsand teach and train others.
  • Written communication skills to accurately document patientinformation such as case notes, treatment and care plans, and to producewritten reports and job-related correspondence.

The current salary range (Pay Range 2IN) is$78,021-$86,854annually,and the resident incentive salary range for City of Milwaukee residentsis$80,361-$89,460annually.Appointment above the minimum ispossible based upon level of experience and other qualifications and is subjectto approval.
