The Public Health Nurse performs professional and technical work as a generalist supporting clinic coordination, program development and client care. The role provides assessment, clinical services, collaboration of preventive health programs, referrals and outreach to the community with a health equity focus. Work involves providing client care and education to individuals, families, and groups on various public health topics and health priorities, both in a clinic setting and at outreach events. Priority work includes addressing client and community partner needs, supporting physical and behavioral health, and utilizing community resources, data and programmatic strategies for the betterment of the community. Nurses may assist with several core public health services and programs, including the Immunization clinic and disease control investigations, support on Maternal Child Health priorities, HCP Program for Children with Special Needs, Reproductive Health clinic, nurse visitation program and general public health priorities depending on the needs of the department

Salary Range: $37.81 - $51.15/hour

Essential Required Tasks/Examples Of Duties

  • Perform health assessments of clients, identify health care needs and resources, and implement health care plans and/or referrals for clients as needed.
  • Maintain, develop and coordinate clinic processes, protocols, inventory, ordering and supplies as needed.
  • Assist with outreach events and provide education and resources to the community and external partners.
  • Assist with public health calls and prioritizing tasks. Respond, as required, to support public health emergencies, incidents and events.
  • Provide programmatic, technical assistance and training to community agencies and providers including schools, child care centers and other community stakeholders.
  • Collaborate with stakeholders in other Health and Human Services divisions, county agencies, community agencies, non-profit organizations, and vendors to effectively serve the needs of the citizens.
  • Assist the Nursing Manager in the preparation of the department annual budget, appropriate fiscal spending and overall programmatic development.
  • Assist in process improvements and collaboration with other internal and external teams to implement program deliverables and public health priorities as needed.
  • Keep advised of updates and department priorities, as well as pertinent new rules, regulations, legislation, procedures, and professional developments
  • Maintain confidentiality of information consistent with applicable federal, state and county rules and regulations.
  • The ideal candidate will thrive in a fun work environment with a variety of responsibilities and will exhibit flexibility while working on multiple programs in a small office setting.

Other Duties And Requirements

  • May assist with communicable disease cases/outbreaks by being a vital link between epidemiology and clinical application.
  • Strong communication skills and ability to build trust with internal and external partners as a subject matter expert supporting all aspects of public health.
  • Participate in improvement strategies for public health and assure that responsibilities are performed in compliance with CCOB policies and procedures.

REPRODUCTIVE HEALTH CLINIC SPECIFIC REQUIREMENTS (if Position Dictates)

  • This position supports the Advanced Practice Nurse clinician (nurse practitioner/ midwife) in providing patient care.
  • Conducts in-house testing including urine dip, rapid HIV, rapid syphilis. Performs applicable laboratory work/draws.
  • Acts as the primary provider for our Walk-in clinic for. STI screening and supply pick-up, on Thursday mornings.
  • Job duties include direct contact with the public, external partners and clinical patient care.
    • Administers day to day activities of clinic programs, provides patient education,referrals and safety as appropriate.
    • Performs health assessments of clients, identifies health care needs and resources, and develops effective treatment plans for clients.
    • Provides care coordination for clients and assists with identifying community resources, early intervention education and referrals.
    • Document clinic visits in EMR (Athenahealth is used currently)
    • Operate and maintain clinic equipment: Autoclave-package and sterilize instruments; glucometer
  • Assists with clinic/pharmacy/lab inventory, and maintains clinic and lab data.
  • Adheres to and helps develop program policies and protocols.
    • Operates a personal computer including associated software programs.
    • Uses EMR to record client encounters (Athenahealth is used in the clinic)
    • Assist with creating and implementing additional public health programs.
    • Provides community outreach for the Reproductive Health Clinic as directed.
    • Attends meetings, workshops, and conventions as necessary for the maintenance of effective services.
  • May assist with other health clinics as necessary; Performs related duties as required.
  • Clinic days are generally Tuesday and Wednesday 8:00 AM–5:00 PM, and overflow on Thursday 8:00 AM-!2:00 PM. Hours may vary depending on community activities, training, and when we need to cover for a team member on vacation.
Minimum Qualifications

Education

  • Bachelor's degree in nursing from an accredited college or university.

Experience

  • 2+ years of increasingly responsible nursing experience, preferably in public health, with some responsibility in clinical support including immunizations and/or reproductive health services and/or implementation of community health programs
  • Bi-lingual skills including Spanish are helpful but not required

OR

Years of related experience may be substituted for required education, however education cannot be substituted for the necessary work experience. Candidates must meet the minimum required years of work experience

Necessary Special Requirements: Licensure to practice professional nursing in the State of Colorado. BLS certification required (CPR/First Aid/AED). Must be at least 21 years of age. Must possess and maintain a valid driver’s license and safe driving record for continued employment. New hires must, as a condition of employment, pass the following pre-offer and post-offer/hire processes: oral board, reference checks, background checks including local police check, child abuse and sex-offender registry, CVSA or polygraph, and driving check through DMV

Working Conditions

The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of the job. The noise level in the environment is moderate. The work schedule is varied and the incumbent must be available to work any day of the week including evening, weekend and holiday hours. The incumbent must be on-call and available to respond to emergencies within a reasonable time frame with little or no notice. The incumbent may meet with clients in their homes or other environments, requiring travel by motor vehicle. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of the job. While performing the duties of this job, the employee is frequently required to sit, talk or hear, walk and use hands to finger, handle, or feel objects, tools, or controls. The employee is occasionally required to reach with hands and arms, stand, climb or balance, stoop, kneel, crouch, or crawl. Physical demands are generally described as medium (exert up to 50 lbs. of force occasionally and/or up to 20 lbs. of force frequently, and/or up to 10 lbs. of force constantly to lift, carry, push, pull, or otherwise move objects, including the human body). The employee must be able to observe safe practice criteria in assisting clients. The employee must be able to safely support the full weight of client when necessary. Travel by automobile is required to attend home visits, court, and meetings. Conducting home visits may involve carrying a case filled with materials such as a small DVD player, files, notebook, educational materials, toys, a baby scale, etc., and walking up and down stairs frequently when no elevator is available. Specific vision abilities required by this job include close vision, distance vision, ability to adjust focus, and peripheral vision.
