Schedule: Monday- Friday 8am -5 pm

Setting: Dental Office

Procedures/Cases: General Dentistry

Credentialing Timeframe: 60 days

Electronic Medical Record (EMR) Epic

Requirements:

• Basic Life Support (BLS)
• Controlled Substance Registration (CSR)
