Schedule: October coverage needed

Procedures/Cases: General Dentistry

Credentialing Timeframe: 1-2 days

Requirements: Active State License, Drug Enforcement Administration (DEA), Cardiopulmonary Resuscitation (CPR), Basic Life Support (BLS), and Active Medicaid
