Job summary

Washington State Department of Social and Health Services seeks a PBS4 – CPSS to join the Columbia River Community Service Office in Vancouver, WA. This full-time, in-office role leads client-centered case planning, eligibility assessments, and service coordination with healthcare, housing, employment and more.

You will assess eligibility across cash, food, and medical programs, develop tailored plans, and partner with agencies to remove barriers.

Qualifications

• Eligibility knowledge for Washington’s public assistance programs.
• Ability to read and apply WAC/RCW and internal procedures.
• Trauma-informed case planning aligned with client context.
• Strong data interpretation and documentation skills.

Responsibilities

• Conduct detailed interviews to assess eligibility for cash, food, and medical programs.
• Facilitate program orientations and explain benefit pathways empathetically.
• Design and update personalized case plans to address barriers and employment goals.
• Refer clients to housing, health, and employment services as needed.
• Coordinate with external agencies to align support plans.
• Review complex documents to determine program fit (court orders, leases, medical notes).
• Authorize and track services like transportation, childcare, or work clothing.
• Maintain accurate case notes in ACES, Barcode, eJAS, HPF.
• Respond to audits and ensure compliance with federal/state regs.
• Engage in outreach, home visits, and client advocacy.
