Job summary

Adoration Health is seeking a Home Health Physical Therapist Assistant to join our Savannah, GA team. You will deliver one-on-one PTA services in patients' homes, working under a Physical Therapist to implement plans of treatment.

The role emphasizes independence with team support, flexible scheduling, and opportunities for growth within a growing home health organization.

Qualifications

• Current unrestricted PTA license in the applicable state.
• BLS CPR certification is required.
• Minimum 1 year PTA experience preferred.
• Knowledge of rehabilitation techniques for complex neurological injury.

Responsibilities

• Provides 1:1 home health physical therapy services per Plan of Treatment.
• Regularly confers with Physical Therapist regarding plan and progress toward goals.
• Assists in obtaining adaptive equipment to improve function and uses it in rehab.
• Maintains and submits patient treatment and billing documentation on time and per regulations.

Office Location: Savannah, GA

Coverage area: Liberty, Bryan, Chatham, Effingham

Schedule: Monday-Friday 8:00am-5:00pm

Responsibilities

As a Home Health PTA, You will:

• Provides physical therapy services planned, delegated and supervised by the qualified Physical Therapist in accordance with the patient's Plan of Treatment
• Confers with Physical Therapist regularly regarding patient's plan of treatment, status of progress toward goals, questions and concerns
• Communicates patient's needs and progress to the Physical Therapist of record and assists in securing special adaptive equipment for patient to improve function. Incorporates the use of adaptive equipment to enhance the rehabilitation program
• Maintains and submits patient treatment and billing documentation in the format and timelines required by the Company, facility, funding source, local, state and federal regulations

Qualifications

• Current unrestricted license as a Physical Therapist Assistant by state in which practicing
• Current Basic Life Support or cardiopulmonary resuscitation certification, in accordance state regulations
• A minimum of one year's work experience as a Physical Therapist Assistant preferred
• Demonstrates knowledge of rehabilitation techniques related to complex neurological injury
