Job Description

We are seeking a dedicated Outpatient Physical Therapist to evaluate and treat assigned patients in accordance with their needs, goals, and physician direction. The therapist will be responsible for developing, implementing, and modifying treatment plans while ensuring a focus on functional treatment programs for successful community reintegration.

Responsibilities
  • Conduct comprehensive evaluations of patients and create individualized treatment plans.
  • Complete medical record documentation accurately and timely, following compliance guidelines.
