Plaza Health & Rehab (HVH) is seeking a Physical Therapist Assistant to support resident rehabilitation under a supervising Physical Therapist. You will implement individualized plans and work with the rehab team to help residents improve mobility and independence.

The role emphasizes teamwork, compassionate care, and ongoing training in a positive work environment with a focus on resident-centered outcomes.

Formación

• Accredited Physical Therapist Assistant program; licensed or eligible per state requirements.
• Strong organizational and time-management skills.
• Excellent verbal and written communication abilities.
• Compassionate, dependable, team-oriented approach to resident care.

Responsabilidades

• Implement individualized treatment plans under supervision of a Physical Therapist.
• Collaborate with rehabilitation team to tailor resident programs.
• Assist the PT with tests, evaluations, and procedures as directed.
• Instruct residents, families, and caregivers in PT techniques under supervision.
• Support residents to improve independence and functional abilities through patient-centered care.
