Plaza West Healthcare & Rehab Center seeks a dedicated Physical Therapist Assistant to support the licensed Physical Therapist in implementing patient treatment plans. You will track progress, educate residents and caregivers, and ensure accurate documentation in compliance with regulations.

Qualifications include an associate degree from an APTA-accredited program and an active PTA license. This role involves standard PT setting duties with a focus on patient-centered care in a facility

Qualifications

• Associate degree from an APTA-accredited program.
• Licensed as Physical Therapist Assistant.

Responsibilities

• Provides direct treatment to residents under the plan of care outlined by the supervising Physical Therapist.
• Documents resident progress toward meeting established goals.
• Teaches exercises to residents, family members, and caregivers; may instruct in use and care of orthotic, prosthetic devices, and other assisting devices.
• Completes required forms and documents per company policy and state/federal regulations.
