As a Physical Therapist Assistant in our Skilled Nursing setting, you will evaluate and treat geriatric patients, create individualized plans, and collaborate with an interdisciplinary team to improve functional outcomes.

Qualifications

Associates degree from an accredited PT program.
Connecticut PTA license or eligibility is required.
Interest in geriatric rehabilitation and functional outcomes.
Strong communication and teamwork skills are essential.

Responsibilities

Evaluate and treat patients recovering from orthopedic surgeries, neurological events, cardiac conditions, and deconditioning.
Develop individualized treatment plans and document progress.
Collaborate with an interdisciplinary care team to achieve functional outcomes.
Support patient return to highest possible independence.
