Tender Touch Rehab Services is seeking a licensed Physical Therapist Assistant for a full-time role in a SNF/Long-Term Care setting. You will provide therapy under a licensed PT, identify appropriate patients, and coordinate referrals to support outcomes.

Formación

• Must be a licensed Physical Therapist Assistant in the state of practice.
• Experience in SNF or long-term care settings preferred.
• Strong communication and organizational skills are essential for patient care and team coordination.

Responsabilidades

• Provide physical therapy treatments under the supervision of a licensed Physical Therapist, following state practice guidelines.
• Assist in identifying appropriate patients for therapy and coordinate with the PT for evaluation and referral.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet all regulatory requirements.
• Ensure all required physician orders are obtained and up to date.
