Job summary

A Path of Care Home Health is seeking a dedicated Physical Therapist Assistant to provide skilled home health care in the Tulsa, OK area. You will work under the supervising Physical Therapist to help patients improve mobility, strength, and independence in their homes.

You will assess and implement treatments, teach safe exercises to patients and caregivers, and coordinate with the home health team. A current Oklahoma PTA license, one year of experience, and CPR (or ability to obtain within 60

Qualifications

• Successful completion of an accredited physical therapy assistant program.
• Current Oklahoma PTA license in good standing.

Responsibilities

• Providing treatments planned, delegated, and supervised by the evaluating Physical Therapist.
• Assisting with therapeutic exercises and functional mobility training.
• Teaching patients and caregivers how to safely perform exercises and activities.
• Coordinating care with other home health team members to ensure well‑rounded treatment.
• Maintaining accurate documentation in accordance with CMS requirements, including functional reassessments.
• Practicing infection control and universal precautions.
• Reporting suspected abuse according to state and federal guidelines.
• Assisting with new-hire orientation and participating in training activities when needed.
