Fair Oaks Health and Rehabilitation is seeking a dedicated Physical Therapist Assistant to join our in-house therapy program in Jamestown, KY. You will work under a licensed physical therapist to deliver personalized treatment plans focused on improving strength, mobility, and confidence for residents.

You will collaborate with an interdisciplinary team, support supervision of rehab staff, and participate in care conferences, documentation, and program development within facility guidelines.

Qualifications

• Graduate of an accredited Physical Therapy Assistant program.
• Valid state PTA license.
• Proficiency with Microsoft Office applications and basic internet navigation.

Responsabilités

• Reports to the Director of Rehabilitation and Physical Therapist.
• Assist in screening and providing input to initial evaluations as appropriate.
• Implement treatment for all assigned patients in accordance with the established plan of care.
• Comply with state and federal guidelines regarding accurate and timely documentation and billing.
• Establish and maintain effective patient-centered interactions with patients, interdisciplinary team members, and facility staff.
• Provide direct supervision to rehabilitation technicians during group and individual treatment.
• Actively integrate physical therapy services into facility and interdisciplinary treatment plans.
• Participate in utilization review, case management, care conferences, administrative meetings, and family conferences as directed by the physical therapist.
• Establish and maintain effective working relationships with healthcare professionals and colleagues.
• Make appropriate referrals to other members of the rehabilitation team.
