Job summary

Shelby Skilled Nursing and Rehabilitation in Sidney, Ohio is seeking a dedicated Physical Therapist Assistant to join our in-house therapy team. You will support residents in regaining strength, mobility, and independence, helping them return home or adjust to new living arrangements.

You will collaborate with licensed therapists and nursing staff to deliver individualized treatment plans, monitor progress, and maintain precise documentation in a fast-paced skilled nursing setting.

Qualifications

• Licensed Physical Therapy Assistant (Required).
• Geriatrics experience a plus, but not required.
• Strong communication and interpersonal skills.
• Ability to work in a fast-paced environment and prioritize tasks effectively.
• Compassionate and patient-focused approach to healthcare.

Responsibilities

• Assist licensed physical therapists in developing and implementing individualized treatment plans.
• Conduct physical therapy sessions, including exercises and modalities, to address rehabilitation goals.
• Monitor and record patient progress, adjusting treatment plans as needed.
• Collaborate with nursing staff and other healthcare professionals for a comprehensive approach to care.
• Maintain accurate records and documentation of treatments and progress.
