Job summary

Home Health Care, Inc. is seeking a Minnesota-licensed Physical Therapist Assistant (PTA) to provide one-on-one therapy in patients’ homes across St. Cloud and surrounding communities. You’ll travel to clients, implement individualized treatment plans, and support functional recovery in a home-health setting.

The role emphasizes independent day-to-day management while staying connected to supervising therapists and the care team, with a focus on safe mobility and meaningful outcomes.

Responsibilities

• Provide one-on-one Physical Therapy treatments in patients' homes.
• Carry out individualized plans of care.
• Provide therapeutic exercise and strengthening.
• Perform gait and mobility training.
• Educate patients and caregivers on safe mobility and techniques.
• Monitor patient response and communicate changes in condition.
• Update supervising PT on patient progress.
• Collaborate with nurses, OTs, SLPs, physicians, and other clinicians.
• Complete timely and accurate clinical documentation.

What You'll Do

Working under the direction of the supervising Physical Therapist, you'll:

• Provide one-on-one Physical Therapy treatments in patients' homes

• Provide therapeutic exercise and strengthening
  
• Perform gait and mobility training
  
• Improve balance, coordination, transfers, and endurance
  
• Educate patients and caregivers on safe mobility and therapeutic techniques
  
• Monitor patient response and tolerance to treatment
  
• Recognize and communicate changes in condition
  
• Update the supervising PT on patient progress
  
• Collaborate with nurses, OTs, SLPs, physicians, and other interdisciplinary clinicians
  
• Complete timely and accurate clinical documentation
