Job summary

Sequoia Transitional Care – Porterville, CA is seeking a compassionate Physical Therapist Assistant (PTA) to join our rehabilitation team. Under the supervision of a licensed Physical Therapist, you will implement individualized treatment plans, monitor progress, document care, and collaborate with an interdisciplinary team to help residents regain function and independence.

Qualifications

• Associate degree from an accredited PTA program.
• Experience in skilled nursing/rehabilitation preferred.
• Experience with Net Health and PointClickCare preferred.

Responsibilities

• Provide physical therapy treatments under supervision.
• Complete timely, accurate documentation per policies.
• Maintain a safe treatment environment following infection control guidelines.
• Assist with rehabilitation team meetings and discharge planning.
• Educate residents and caregivers on home exercise and safety.
• Communicate resident progress and treatment goals with supervisor.
