Qualifications

Responsibilities

• Provide safe, effective therapy under licensed PT supervision.
• Progress patient care plans established by the PT.
• Document progress notes, discharge summaries, and SOAP notes.
• Communicate with supervising PT about treatment plans and discharges.
• Supervise rehab aides and coordinate with staff as needed.
• Engage in community services and screenings as appropriate.
• Maintain patient charts and insurance/plan documentation.
