Job summary

Tender Touch Rehab Services seeks a compassionate Physical Therapist Assistant to provide high-quality rehab services to geriatric and sub-acute residents in skilled nursing facilities.

Working under the direction of a Physical Therapist, you will carry out individualized treatment plans, including therapeutic exercise, gait and balance training, and functional mobility activities to improve safety and independence.

Qualifications

• Experience providing rehab services to geriatric/sub-acute populations
• Ability to follow treatment plans under Physical Therapist supervision
• Strong collaboration with interdisciplinary teams to support patient care

Responsibilities

• Carry out individualized treatment plans including therapeutic exercise, gait and balance training, and functional mobility activities
• Document progress and educate patients and families on care and progress
• Collaborate with interdisciplinary team to support a patient-focused culture of clinical excellence
