Job summary

Lebanon Health and Rehabilitation Center is seeking a Physical Therapist (PT) for a PRN weekday position. The PT will assess patients, design and administer therapy plans, set achievable goals, and document progress while educating families.

The role includes supervising assistants and coordinating with a multidisciplinary team to deliver high-quality geriatric care. Applicants must be licensed, with completion of an accredited PT program.

Qualifications

• Graduated from an accredited Physical Therapy college or university.
• Must hold a valid professional license and state registration.
• Oversee and supervise Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care preferred.
• Strong documentation and record-keeping skills.
• Complete evaluations, weekly progress notes, recertifications and discharge documents.
• Collaborate with disciplines to provide patient-centered care.
• Maintain confidentiality and HIPAA compliance.
• Experience with Case Mix Index and PPS scheduling preferred.

Responsibilities

• Assess referred patients needing rehabilitation and provide direct patient care.
• Establish and administer treatment programs with goals aligned to patient capacity.
• Set realistic goals, document patient condition, and educate patients and families.
• Ensure timely progress notes, recertifications and discharge documentation.
• Collaborate with other disciplines to deliver best patient-centered care.
• Supervise Physical Therapy Assistants and ensure professional conduct.
