Job summary

Wellington Parc Nursing & Rehab is seeking a Physical Therapist (PT) to join our team focused on geriatric rehabilitation within a skilled nursing facility. The role emphasizes evaluating, planning, and implementing personalized PT treatments to improve mobility, strength, balance, and independence.

The PT will collaborate with an interdisciplinary care team to ensure resident-centered care, uphold HIPAA guidelines, and promote safety and well-being.

Qualifications

• Doctorate or Master’s degree in Physical Therapy from an accredited program.
• Current state licensure as a Physical Therapist.
• Minimum 2 years of experience in physical therapy, preferably in a long-term care or geriatric setting.

Responsibilities

• Conduct initial and ongoing evaluations of residents’ physical therapy needs, including mobility, strength, balance, and functional limitations.
• Develop and implement individualized treatment plans based on clinical findings and resident goals.
• Provide direct physical therapy interventions such as therapeutic exercises, gait and balance training, mobility enhancement, and manual therapy.
• Utilize evidence-based practices to promote functional outcomes and independence.
• Monitor and document residents’ progress, adjusting treatment plans as needed.
• Educate residents, families, and caregivers on treatment plans, home exercises, and safety strategies.
• Collaborate with nursing, occupational therapy, and speech therapy to ensure coordinated care.
