Job summary

Continuum Therapy Partners, LLC is seeking a Physical Therapist to join our therapist-led network. This full-time daytime role is based at our Browns Valley/Morris Health Services locations in MN, with a 13-week travel assignment option available to support staffing needs.

Candidates should have a Doctor of Physical Therapy (DPT), an active state PT license, and NPTE eligibility. Prior PT experience is preferred, with emphasis on developing and adjusting patient treatment plans and educating

• 1 year of PT experience preferred.
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist - $2,406 Weekly Gross

Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
