Job Description

$5,000 Sign on Bonus for external candidates only with a one year commitment to the organization.

Position Summary

Develops and administers physical therapy treatment programs to facilitate rehabilitation of ill, injured or physically disabled patients to maximize function and independence. Directs patient participation in selected tasks to restore, reinforce and enhance performance. Facilitates the learning of those skills and functions essential for adaptation and productivity.

Accountabilities

1. Conducts physical therapy evaluations using age-specific criteria; demonstrates skill in assessment and interpretation.
2. Develops and implements treatment plans based on valid prescriptions, considering patient age and educational level.
3. Prepares and maintains accurate patient care records, ensuring confidentiality.
4. Maintains a clean and safe work environment in compliance with safety standards.
5. Participates in continuing education and shares information with staff, demonstrating professionalism.
6. Communicates and educates patients of all ages, considering physical, emotional, language, and cognitive impairments.
7. Uses equipment appropriately, following age-related recommendations and guidelines.
8. Maintains departmental productivity standards.
9. May assist in supervising assistants, students, aides, and volunteers.
10. Participates in program development and performance improvement initiatives.
11. Performs other duties as assigned.

Job Requirements

Education: Master’s Degree or Doctorate in physical therapy from an accredited program.

License: Licensed in Ohio and/or Michigan to practice physical therapy.

Certification: CPR certification per hospital policy.
