Job summary

Elderwood Skilled Nursing & Rehabilitation in New York is seeking a compassionate Physical Therapist (PT) to join our rehabilitation team. This role focuses on restoring mobility and independence for older adults in skilled nursing settings.

You will work in an interdisciplinary environment with individualized treatment plans and participate in care planning, helping residents progress beyond productivity metrics while using modern therapy equipment and supportive teams.

Qualifications

• Graduation from an approved school of physical therapy.
• Active state license to practice physical therapy.
• Two years' experience preferred.
• Ability to direct others is necessary.

Responsibilities

• Provide skilled physical therapy evaluations and treatments in accordance with physician orders
• Develop and update individualized care plans and therapy goals
• Maintain timely and accurate documentation
• Collaborate with interdisciplinary clinical teams regarding resident progress and needs
• Participate in resident care planning meetings
• Assist with education and training initiatives as needed
• Support a safe, respectful, resident-centered care environment
