Elderwood Skilled Nursing & Rehabilitation is seeking a compassionate Physical Therapist (PT) to join our rehabilitation team at Elderwood at Liverpool. This role focuses on restoring mobility, creating individualized treatment plans, and documenting progress within an interdisciplinary setting.

Qualifications

  • Graduation from an approved school of physical therapy.
  • State license to practice physical therapy.
  • Two years’ experience in physical therapy preferred.
  • This position requires that the employee read, write, speak and understand English at an intermediate or higher level.

Responsibilities

  • Provide skilled physical therapy evaluations and treatments in accordance with physician orders.
  • Develop and update individualized care plans and therapy goals.
  • Maintain timely and accurate documentation.
  • Collaborate with interdisciplinary clinical teams regarding resident progress and needs.
  • Participate in resident care planning meetings.
  • Assist with education and training initiatives as needed.
  • Support a safe, respectful, resident-centered care environment.

Qualifications

Physical Therapist (PT)

  • State license to practice physical therapy required.

  • Ability to direct others necessary.
  • This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
