Adoration Health is seeking a Part-time Home Health Physical Therapist to join our Salisbury, MD team. You will provide patient-centered care in a home setting while coordinating with physicians and families to support independence and function.

The role requires a PT license, CPR certification, and 2+ years of experience; new graduates may be considered in select markets. Expect a supportive team, flexible scheduling, and opportunities for professional growth within a growing company.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• Minimum 2 years of appropriate experience as a physical therapist; hospice/community experience preferred.
• Valid license in the state of practice and CPR certification.
• New PT graduates may be considered in select markets based on program availability.

Responsibilities

• Perform physical therapy evaluation and treatment of patients based on physician orders, including adaptive equipment recommendations.
• Establish measurable goals and develop treatment plans to move patients toward independence.
• Maintain communication among the treatment team, physician, patient, and family members.
• Maintain thorough and timely documentation of services for proper billing.
• Supervise PTs and PT aides in direct patient care as applicable.

Responsibilities

• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Minimum of Bachelors Degree in Physical Therapy from a college or university with accredited Physical Therapy program
• New physical therapy graduates may be considered in select markets based on program availability
• At least (2) years of appropriate experience as a physical therapist; community/hospice experience is preferred
• Valid license from the state of practice
• Possess and maintain current CPR certification
