Job summary

Adoration Health is seeking a Home Health Physical Therapist to join our De Queen, AR team in a PRN capacity. You will deliver 1:1, patient-centered care with independence and strong team support.

You will perform PT evaluations and treatments per physician orders, set measurable goals, document thoroughly, and collaborate with patients and families. We offer Medical/Dental/Vision, 401(k), tuition discounts, PTO and wellness programs.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• At least 2 years of PT experience; hospice/community preferred.
• Valid license in state of practice and CPR certification.

Responsibilities

• Perform PT evaluation and treatment per physician orders, including adaptive equipment.
• Establish measurable goals and treatment plans to maximize independence.
• Maintain communication with the treatment team, physician, patient and family.
• Maintain thorough documentation for proper billing.
• Supervise PTAs and PT aides in patient care as allowed by state act.

Responsibilities

As a Home Health Physical Therapist , You w ill:

• P erform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Minimum of Bachelors Degree in Physical Therapy from a college or university with accredited Physical Therapy program
• New physical therapy graduates may be considered in select markets based on program availability
• At least (2) years of appropriate experience as a physical therapist. Community/hospice experience is preferred
• Valid license form the state of practice
• Possess and maintains current CPR certification
