Job summary

Adoration Health, an affiliate of BrightSpring Health Services, is seeking a Home Health Physical Therapist for Monroeville, AL. The role focuses on 1:1 patient care in a home health setting with flexible PRN scheduling.

You will evaluate, treat, and collaborate with families and clinicians to maximize patient independence. A valid PT license and CPR are required; new grads may be considered in select markets.

Qualifications

• Minimum Bachelor’s degree in Physical Therapy.
• New PT graduates may be considered in select markets.
• Valid license to practice physical therapy.
• CPR certification current.

Responsibilities

• Perform PT evaluation and treatment per physician orders.
• Set measurable goals and develop treatment plans.
• Maintain communication with treatment team, physician, patient and family.
• Document services timely for accurate billing.
• Supervise PTAs and PT aides per state act.

Responsibilities

As a Home Health Physical Therapist, You will:

• Perform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient‑related activities in accordance with state practice act

Qualifications

• Minimum of Bachelors Degree in Physical Therapy from a college or university with accredited Physical Therapy program
• New physical therapy graduates may be considered in select markets based on program availability
• At least (2) years of appropriate experience as a physical therapist. Community/hospice experience is preferred
• Valid license form the state of practice
• Possess and maintains current CPR certification
