Job summary

Adoration Health, an affiliate of BrightSpring Health Services, is seeking a dedicated Physical Therapist for Home Health in Gadsden, AL. You will provide 1:1 patient care in a home health environment with a focus on improving independence and function.

The role offers full-time hours, collaboration with a supportive team, and opportunities for professional growth within a growing company. State licensure and CPR certification are required.

Qualifications

• Graduate of CAPTE-approved Physical Therapist program.
• Licensed PT in the state and CPR certification.
• 2 years of adult PT/home health experience preferred.

Responsibilities

• Perform physical therapy evaluation and treatment per physician orders, including adaptive equipment.
• Establish measurable goals and treatment plans toward independence and function.
• Maintain communication among the treatment team, physician, patient and family.
• Document services thoroughly for proper billing.
• Supervise PTAs and PT aides per state practice act.

Responsibilities

• Perform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Graduate of a Physical Therapist Education program approved by CAPTE
• Current Physical Therapist licensure in State and American Heart Association CPR certification required; CPR certification may be offered and completed upon hire
• Two (2) years of experience in adult physical therapy, home health experience preferred
• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; physical therapy skills per competency checklist
