Job Description :

We are seeking a dedicated and skilled Home Health Physical Therapist (PT) to join our team. This role involves providing essential rehabilitation services to patients in the comfort of their homes. As a Home Health PT, you will work one-on-one with a diverse patient population, including adults and geriatrics, to help them achieve their rehabilitation goals.

Key Responsibilities :

• Deliver personalized physical therapy services to patients in their homes, focusing on recovery and wellness.
• Conduct thorough assessments and create tailored treatment plans based on patients' individual needs.
• Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes.
• Educate patients and their families on therapeutic exercises and home care strategies to support long-term recovery.
• Maintain accurate and timely documentation of patient care and progress using the EPIC Remote Client for Home Health and Oasis tools.

Qualifications :

Minimum of 1 year of skilled home health experience required.

Current Basic Life Support (BCLS) certification.

Familiarity with OASIS documentation is mandatory.

Strong communication and interpersonal skills to effectively interact with patients and their families.

Reliable transportation to travel to patient locations; occasional travel may extend up to a 60-mile radius.

Desired Skills :

Experience using electronic charting systems, specifically EPIC.

Proficiency in provided patient care techniques, including but not limited to wound care, adaptive devices, and various therapeutic practices.

Ability to work independently and manage time effectively in a home health setting.

Additional Information :

This position requires a flexible approach to patient care and the ability to drive to various points of service efficiently.

Weekends are not required, and there is no on-call duty.
