Job summary

Trilogy Health Services is seeking a Physical Therapist in Evansville, IN, to deliver comprehensive PT services including assessment, treatment planning and therapeutic interventions. You will work within an interdisciplinary team to enhance patient function and independence.

The role requires a Master’s degree and active PT licensure, with travel as needed. Training and orientation support are provided to help you succeed in the blue-badge period and beyond.

Qualifications

• Master's degree required for PT licensure.
• Individual must be licensed as a Physical Therapist in the state.
• Ability to develop and implement treatment plans.
• Ability to educate patients and families on exercises and safety.
• Maintain accurate documentation in line with policies.

Responsibilities

• Perform thorough assessments of patients’ physical conditions and rehab needs.
• Develop and implement personalized treatment plans and interventions.
• Educate patients and families on exercises, posture and body mechanics.
• Maintain accurate progress documentation and outcomes per regulations.
• Collaborate with an interdisciplinary team to ensure comprehensive care.
• Perform other duties as assigned.
