Job summary

Total Care Therapy in Grand Rapids is seeking a compassionate Physical Therapist to evaluate, develop, and implement treatment plans that improve mobility and independence for residents in assisted living and independent living communities.

You will collaborate with a multidisciplinary team, supervise PTAs, document in EMR, and provide education to residents and staff while growing with a therapist-owned company expanding in Michigan.

Qualifications

• Active State PT license in Michigan.
• CPR/BLS certification.
• Valid driver's license and transportation.
• Experience in geriatrics, home health, or assisted living preferred but not required.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments, including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on treatment plans, home exercise programs, fall prevention, and safe mobility techniques.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering exceptional therapy services.
