Job summary

Qualifications

• Master’s Degree in Social Work from an accredited graduate program.

• California BBS Associate Clinical Social Worker (ASW) - REQUIRED

Responsibilities

• Collaborates with the treatment team to develop competency and acquire technical skills.
• Communicates with the health care team and patient advocates to expedite patient progress while upholding patient rights.
• Develops an appropriate and timely discharge plan and coordinates follow-up care.
• Assesses, evaluates, and documents the psychosocial needs of the patient and family; develops a plan of care.
