Responsibilities

• Implements counseling/therapy for assigned patients in individual and group settings.

• Assists with discharge planning and identifies community resources as part of the plan.
• Collaborates with care team on discharge readiness and needs.
• Uses teaching methods and referrals to meet patient/family educational needs.
• Trains new staff, students and interns as assigned.
• Works with critical thinking, decisive judgment, and minimal supervision.
• Maintains performance in a fast-paced environment.
