Jade Financial seeks a dedicated psychosocial rehabilitation professional to provide group and individual services in a long-term care setting in Chicago, IL. You will collaborate with the interdisciplinary team to develop, implement, and evaluate therapeutic activities for residents.
Responsibilities include leading socialization groups, assisting with meals and daily operations, maintaining accurate records, and supporting residents during community outings.
The primary purpose of this position is to provide group psychosocial rehabilitation services to adults within a long term care/nursing home setting. This position participates with the interdisciplinary team to develop, implement and evaluate effective therapeutic services via facility programming.
